🌿 What Is Phalloplasty?

Masculine Reconstruction Surgery in Thailand

Phalloplasty is a gender-affirming surgical procedure that creates a penis using a person’s own tissue—most commonly from the forearm, thigh, or abdomen. For many trans men and transmasculine people, it’s a defining step toward aligning body and identity.

At Now Be You (NOWBU), we connect clients with trusted Thai surgeons and hospitals that specialise in advanced gender surgery. Our team supports you at every stage—from your first questions to your final follow-up—so you can focus on healing and self-confidence.

🧭 The Goal of Phalloplasty

Phalloplasty aims to construct a natural-looking, sensate penis that allows:

  • Standing urination (via urethral lengthening)

  • Sexual sensation (through nerve connection)

  • Future erectile or testicular implants (if desired)

Depending on personal goals, additional procedures such as scrotoplasty or glans shaping can be performed in the same or later stages.

🔍 Why Thailand?

Thailand has become one of the world’s leading destinations for gender-affirming surgery thanks to:

  • Experienced multidisciplinary surgical teams

  • Modern private hospitals with international standards

  • Transparent, lower overall costs than in Western countries

  • Established aftercare systems and supportive community networks

NOWBU partners exclusively with hospitals and surgeons who meet these international safety and ethical standards.

⚙️ How the Procedure Works

Phalloplasty is usually completed in three stages over 9–12 months:

  1. Hysterectomy – removal of the uterus and ovaries (2 nights stay)

  2. Vaginectomy + Urethral Lengthening + Forearm Preparation – creates the foundation for the future urethra and phallus (6 nights stay)

  3. Total Phalloplasty (Radial Forearm Free Flap) – sculpts and connects the phallus using microsurgery (14 nights stay)

Each stage allows adequate healing before progressing to the next.

👩‍⚕️ Surgical Expertise

NOWBU works closely with Dr Poonpisamai, renowned for her pioneering work in both MtF SRS and FtM metoidioplasty / phalloplasty. She offers both a multi-stage and an “all-in-one” phalloplasty, which includes vaginal closure, urethral and phallus reconstruction, and scrotoplasty with coronal shaping—all within a single hospital stay of about 7 nights.

💬 Realistic Expectations

Every surgical path is unique. Results vary based on anatomy, donor site, healing, and personal goals. Some clients start with metoidioplasty, achieving 2–3 inches of length without skin grafting, while others proceed directly to phalloplasty for greater size and prosthetic options.

Your NOWBU consultation helps determine which route aligns best with your vision and medical readiness.

💼 Estimated Investment

Typical total costs—including surgery, flights, accommodation, and recovery—range between 43,200 – 49,900 AUD.
This estimate excludes insurance, potential revisions, and companion travel.

🧘 Preparing for Surgery

Success starts with preparation: completing hysterectomy at least two months prior, laser hair removal on the donor site, obtaining two psychiatric letters, pausing hormones two weeks pre-op, and maintaining a BMI ≤ 25.
A minimum four-week stay in Bangkok after surgery is recommended for review and healing.
